Consent for Services
I understand and consent to my child receiving educational, behavioral, and emotional support services through Progressive Point Learning. Services may be provided in home, school, and community-based settings, as appropriate, and may include collaboration with teachers, parents, and caregivers to support my child’s development and success.
Scope of Services
Progressive Point Learning provides supportive services for children with attention, developmental, and learning needs, including but not limited to children with ADHD, Autism Spectrum differences, and learning challenges.
Services may include:
Classroom shadowing and in-school support
Behavioral regulation and emotional support strategies
Task initiation, organization, and completion support
Executive functioning and learning support
Teacher and parent collaboration and coaching
No Formal Diagnosis Disclaimer
I understand that Progressive Point Learning does not provide formal medical, psychological, or educational diagnoses. Services offered are supportive and educational in nature and are not a substitute for diagnostic evaluations, medical treatment, or clinical therapy. Any diagnoses referenced are those provided independently by licensed professionals.
Collaboration & Information Sharing
I authorize Progressive Point Learning to communicate and collaborate with my child’s school, teachers, and caregivers for the purpose of supporting consistency and progress, as appropriate and with my consent.
